There is a solution to this problem and to all the other problems  
that arise because MD can't distinguish between income and cash  
flow.  It should be possible to produce a transaction report for a  
group of accounts selected by the user.  In other words, the user  
could decide which accounts he regarded as his current cash flow, and  
produce reports for those accounts only.  Transfers to or from other  
accounts would then be cash flow items; transfers within the group  
would have no cash flow effect and would be ignored.

Hiccupqueen's problem would be solved simply by including the credit  
card account in the "cash flow" group.

I know it's easy to suggest things and it may be very difficult to  
implement them.  But I truly believe that MD's reporting badly needs  
this feature.
