[moneydance] Moneydance Entry Forms

Hi Robert,

The new forms are present in the screenshots you sent - they have  
just changed (except for reminder transactions).  You'll find the  
entry forms at the bottom of the transaction register.  The new forms  
work almost exactly as the old ones did, but the fields are now lined  
up with their column in the register.
